Complete yours today!   Eligibility: - Male or female - Grade level 9-12 - Scholastic-Athlete* - Diagnosis of life-threatening illness *A scholastic-athlete is a high school level (grades 9-12) student-athlete who competes in a sport sponsored solely by the high school to which they attend or are affiliated.
